2. If you have an HMO insurance plan you will need to make sure you list us (Glowworm Pediatrics - Dr. Vu Nguyen) as your Primary Care Physician (PCP) before your appointment.

3. We do NOT accept out-of-state Medicaid. If you have Medicaid that is outside the state of Texas; you will need to contact Medicaid and determine if changing to an in-state Medicaid is appropriate.
